Our team of Advance Care Practitioners supports patients across East Lancashire to complete a personalised Advance Care Plan (ACP).

An Advance Care Plan (sometimes known as an Advance Statement) sets out a patient’s wishes and preferences regarding their future care. It’s a guide that healthcare professionals, family, and friends can use should a patient become too unwell to make or communicate their own decisions.

Although an ACP has traditionally been associated with life-limiting illnesses, it’s a process that can support a better quality of life for adults at any stage in life, including those who are fit and well.
